If your equipment is dual voltage you'll be OK with only an adapter. If it's not, you'll need a converter and an adapter. If you use only an adapter and you plug a 110v appliance into a 220v circute using only an adapter, there likely will be a little smoke, maybe a little fire and probably a blown circuit. In other words, your applicance will be toast!!

If you look on your appliance it should say 110 volts or 110-220 volts (on a sticker?). If it says 110-220 you can use it in spain with an adapter only. If it says 110 then you will need a voltage converter.
